I tend to agree with your argument here.  Probably issuer identity, and
possibly other metadata, should be part of some "<policy statement>",
that contains policy sets, its issuers and any other related policy
metadata.
Reason for that is that it seems odd to add "issuers" as part of the
policy resolution algorithm.  Policy should resolve to the same
deterministic result independent of who, when and how wrote it.
