Regarding getContentChanges:
getContentChanges would only apply to the CMIS local record. That is; all of the metadata that is contained in the cmis document which includes the string value of the external reference.   e.g. If the external reference changed from http://a/b.doc to http://a/c.doc.  That would be a single string value change nothing more.  Any changes to the payload C.doc are opaque to the CMIS repository and only meaningful to the client that knows how to consume that resource and the server that is hosting that content.  Put another way, the external content is just a string value to CMIS.  It may not even be reachable from the CMIS server.  
 
Regarding indexing:
Attempting to index external content would generally not be recommended.   That would be more of like an internal-google sort of index which is outside the scope here.  This is external content. Everyone must understand that it is not to be treated as if it were the same as regular cmis content.  That is why I am placing it in a distinctly separate property.
