I have implemented Don's contribution.  There are questions, and one major 
issue, that we must address.

ISSUE:  All of Ch.2 is *informative*.  So this contribution places no 
*normative* requirements on implementations.  I assume we intend GZIP 
support to be required, yes?  So where do we add it?  Ch.3?  Ch.7 as a 
conformance requirement on 2.1 interpreters?
