Eclipse UA statisfies all this beautifully and even allows you to have
new content appear within existing sections (a bottom-up toc), but also

I hadn't seen that one. It looks promising.

Anyway, if someone wants to take this up, I'm happy to help out any way
I can. I could certainly provide a very clear description of the general
solution that I think is needed for webapp user assistance :-)
