While the header and footer rules were working with FOP in 1.68.1, other
things related to indents were not.  For example, article tables of

But the use of body.start.indent in FOP will create other indent problems
because of the way the current FOP converts any margin-left into a
start-indent.  Be sure to check your output to see if it exhibits any of the
indent problems.

I have not found a way to overcome the FOP header rule problem.  The rule is
implemented as a bottom border on the header table.  The header table
extends across the page, but for some reaons the bottom rule does not, which
doesn't make any sense.
