Ann, I think the partial answer is when they are declared as such by the parties involved at some recognized point in the process - or are generated as part of some service or action that is recorded. This also spills over into the area and concept of a "digital bag" that the EPR team is currently brainstorming on for e-Healthcare. Another issue is ownership - who is the keeper of this information and why?
