I created and merged a change draft for Issue #389, “toolExecutionSuccessful default too ambiguous”: The problem was that the spec’s description of the default value for
invocation.toolExecutionSuccessful was too confusing, and extended discussion between Michael and James didn’t produce a better answer. Making the property required eliminates all ambiguity about whether the tool succeeded
