The all-lowercase version appears in the attribute declaration, as
Conal says; it also appears in section 4.3, and the PSIs all use the
lowercase version.

The new XTM syntax specification quietly corrects this by using the
lowercase version everywhere:
  <URL: http://www.y12.doe.gov/sgml/sc34/document/0328.htm >

Notice how in appendix B the exact same DTD appears, with the one
difference that the URI has been lowercased. :-)
