I think this table is still not correct.  If the target is
"False" or "Indeterminate", then the "Condition" is not even
evaluated.

I suggest:

      Target        Condition         Rule
      True           True             Effect
      True           False            Not Applicable
      True           Indeterminate    Indeterminate
      False          [not evaluated]  Not Applicable
      Indeterminate  [not evaluated]  Indeterminate
